HNS are approved to provide Disability Related Health Supports under the NDIS. Our senior clinical nurses have the expertise to assess a clients circumstances and design a care plan that meets a clients holistic individual needs.

What is the NDIS?

The National Disability Insurance Scheme (NDIS) is a national system of disability support. It focuses on supporting the needs and choices of those living with disability. The NDIS supports people to live life their own way and also to help them achieve their goals and integrate into social and economic life. 

The NDIS enables people to make decisions about their supports – including who provides them.

We provide initial comprehensive assessment care plan development, personalised clinical services and also deliver a wide range of disability services in both QLD and NSW.

CONTINENCE SUPPORT

There are many things that can be done to manage incontinence and in some cases, incontinence can even be cured. Our friendly nurses are trained to dealing with bowel and bladder issues. 

  • Provide suitably qualified and competent workers to undertake insertion, removal or change of indwelling urethral catheter, suprapubic catheter and/or clean intermittent catheter. 
  • Train suitably qualified and competent workers to provide catheter maintenance and care.
  • Provide daily catheter maintenance and care by a suitably qualified and competent worker.
  • Provide all continence consumables (for example catheter, bags, pads, bottles, straps).
  • Train suitably qualified and competent workers to provide assistance to manage bowel care where needed.
  • Provide suitably qualified and competent workers to provide stoma management and care.

DIABETIC MANAGEMENT SUPPORT

Living with diabetes has its challenges. We are able to help you along this journey so that you can manage and maintain diabetes effectively.

  • Development of a specific diabetes management plan where the client’s disability complicates the management and care of their diabetes. This would build on the clinical plan developed by the General Practitioner.
  • Implementation of a diabetic management plan and daily maintenance and care by an NDIS-funded suitably qualified and competent workers.
  • Training of an NDIS-funded suitably qualified and competent workers to implement a diabetic management plan.

DYSPHAGIA SUPPORT

Dysphagia causes problems when swallowing, chewing, sucking, drinking, eating, dribbling saliva, closing your lips. We are here to assist you with a management plan for dysphagia.

  • Development and periodic review of a meal time management plan by NDIS-funded suitably qualified and competent clinicians.
  • Implementation of a meal time management plan by NDIS-funded suitably qualified and competent workers.
  • Development, implementation and periodic review of an oral eating and drinking care plan (OEDCP) by an NDIS-funded suitably qualified and competent clinicians.
  • Implementation of an OEDCP by NDIS-funded suitably qualified and competent workers.
  • Training of NDIS-funded suitably qualified and competent workers to implement the meal time management plan and OEDCP.
  • Swallowing therapy associated with dysphagia provided by NDIS-funded suitably qualified and competent workers.
  • Consumables such as thickeners to enable a participant to swallow more easily and reduce their risk of aspiration/choking, if not available through the Pharmaceutical Benefits Scheme (PBS).

NUTRITION SUPPORT

Nutrition plays a very important role in maintaining health and vitality and a healthy mindset. We offer Dietetic consultations to help you plan your meals, or set you up with supplements and equipment to ensure you’re set up for a healthy diet.

  • Dietetic consultations and the development and periodic review of a nutritional meal plan by NDIS-funded suitably qualified and competent clinicians.
  • Implementation of a nutritional meal plan by NDIS-funded suitably qualified and competent workers.
  • Training of NDIS-funded suitably qualified and competent workers to implement the nutritional meal plan.
  • Percutaneous Endoscopic Gastrostomy (PEG) Stoma Changes (to assist feeding) by suitably qualified and competent clinicians.
  • Provision of PEG maintenance and care by suitably qualified and competent workers.
  • Training of NDIS-funded suitably qualified and competent workers to provide PEG maintenance and care.
  • Home Enteral Nutrition (HEN) and (PEG) and equipment and consumables excluding food.
  • HEN formula/ nutritional supplements (to meet participant’s nutritional needs) if cost is in excess of PBS funding.

RESPIRATORY SUPPORT

Our suitably qualified and competent clinicians can assist you with care and maintenance plans as well as equipment and consumables for respiratory conditions. 

  • Development and periodic review of a care plan for the daily maintenance of tracheostomy (insertion, removal and change) by NDIS-funded suitably qualified and competent clinicians.
  • Implementation of daily maintenance of tracheostomy (insertion, removal and change) by NDIS-funded suitably qualified and competent workers.
  • Training of NDIS-funded suitably qualified and competent workers to provide tracheostomy maintenance and care.
  • Provision of tracheostomy equipment and consumables.
  • Provision of a Constant Positive Airway Pressure machine (CPAP) machine and consumables.
  • Provision of a Bi-level Positive Airway Pressure (BIPAP) machine and consumables.
  • Provision of a ventilator, air humidifier, portable suction machine and/or cough assist machine and consumables.
  • Assistance with the use of all respiratory medical equipment as well as the training of suitably qualified and competent workers to use the equipment.
Note: the necessary standards and safeguards for provision of respiratory supports is the subject of consideration by the Intergovernmental Clinical Governance Working group. Any requests to the NDIS are being considered on a case-by-case basis in discussion with local health services.

PODIATRY AND FOOT CARE

We are proud to provide high quality podiatry care, delivered with passion. We understand that travelling to your podiatrist may be challenging which is why we offer home visits to our patients. 

  • Custom foot orthotics and ankle-foot orthotics are used to support and align the foot and ankle and improve foot function and posture in general.
  • We may prescribe a foot orthotic for a range of conditions such as foot, ankle, leg and lower back issues.
  • Orthotics are also prescribed to accommodate deformity and reduce spasticity.

WOUND AND PRESSURE CARE

Holistic Nursing Solutions offers training, support, equipment and care plans for chronic wound and pressure care. By proper wound care and education repeat hospital visits can be avoided.

  • Development of a care plan by suitably qualified and competent clinicians. 
  • Wound care and pressure care (including pressure injury management) by NDIS-funded suitably qualified and competent workers.
  • Training of support workers/families in prevention of pressure areas and wounds.
  • Wound care and pressure care consumables (e.g. dressings, gauze, bandages, tape, lymphoedema garments and pressure wraps).
  • Provision of Assistive Technology as required due to a participant’s disability, to prevent pressure areas/wounds occurring.
  • Lymphoedema machines as well as assistance with the use of the equipment and the training of NDIS-funded suitably qualified and competent workers to use the machine.

COUNSELLING

Therapy support is available to you in the comfort of your own home or wherever you want it to be, maybe it’s the coffee shop. Healthy conversation is vital for living a productive and positive life.

HNS counselling and support services assists you in taking care of your mental and emotional health so you can feel happier and enjoy participating in everyday life. Our counsellor will work closely with you to give you tools to cope better with your everyday stresses and challenges. At times you may feel sad, frustrated, angry or lonely and our therapist is here to support you so that you can work through your problems and feel less overwhelmed and more in control.
 
The NDIS provides funding to counselling under ‘Improved Daily Living’ and ‘Community Participation’. If these supports are included in your approved NDIS plan, you should not be out of pocket. 
 
The objective of counselling is to work together with you toward the following outcomes:

  • Building self-worth to bring back self confidence to enable you to engage in daily life activities

  • Implementing a Mindset Board to assist with managing weekly actions and goals
  • Our Counsellor will include added services such as: Individual social skill development, Development of daily living and life skills, Psycho social recovery coaching.
